Seems someone found a seed where the sad ghost doesn’t spawn.

Confirmed by 2 other players, the ghost doesn’t appear. Using a seed checker, it should have spawned on 4th floor. The exit of this floor contain on the the new exit rooms with a stairs down middle of a circular room.

Ok I was right. The ghost spawned in the top left corner of the circular room (CircleBasinExitRoom), outside of the circular area. Thus not reachable as it’s behind the wall. @[email protected]
